New Patient Registration

We will register any patient living in the IG10 postcode area dependent on capacity. New patients must first complete a registration form available from reception. Once you have completed the paperwork please return the pack to reception between 9:00 and 16:00 Tuesday to Thursday so that a receptionist will be able available to photocopy your documents and check through your forms. If you are aged over 16 or over you will also be asked to use our health check machine for blood pressure check and BMI measurement, you may also be asked to attend a new patient medical check once registration is completed.

Your registration will be with the Practice, not with an individual doctor, though should you express a preference we will endeavour to meet your needs.

Medical treatment is available from the date your registration is approved this may take up to 28 days. If you would like further details on essential documentation for registration please call the surgery number and select option 3 to put you through to a member of our staff who will explain the process.

 GP to GP Record Transfers (GP2GP)

It is very important that you are registered with a doctor at all times. When you register with us, your medical records will be removed from your previous doctor and forwarded to us via NHS England. It can take your paper records several weeks to arrive. 

With GP2GP transfers, your electronic record is transferred to your new practice much sooner. 

The Forest Practice confirms that GP2GP transfers are already active and that we send and receive patient records via this system.
